Position Details:

• Hourly Position
• Full-time
• Weekly Schedule Monday through Friday – Hours 8 a.m. to 5 p.m.
• Salary commensurate with experience

Position Summary:

The palliative care Clinical Support Nurse (RN)works as a member of the interdisciplinary team telephonically assisting patients and families in managing the psychosocial, emotional and resource needs relating to life limiting illnesses.

Position Responsibilities:

• Initiating contact with all Keystone patients living in home or assisted living environments as well as other patient/families as identified by the interdisciplinary team.
• Assessing the physical and emotional needs of patients, families and care givers related to the patient's life limiting illness environment.
• Providing support and education for patients, families and community members regarding symptom management, disease process, patient needs, emotional and social aspects of life limiting illness.
• Participating in goals of care discussions with patients and families as needed for patients identified as hospice appropriate.
• Assisting patients with medication refills and communicating symptom management concerns with provider and team.

Position Qualifications:

• Graduate of an accredited school of nursing and current Registered Nurse (RN) licensure in the State of Michigan.
• Two Years of recent clinical experience is preferred. Homecare of hospice experience desired.
• Sound clinical judgment.
• Knowledge and/ or experience in pain symptom management with end of life care.
